Supportive & Community Housing

Pathways supportive and community housing are our more independent options for our clients. Individuals are able to show that they can live on their own and be a part of the community with some support on a need-basis. Each client is given a case manager to make sure they are staying on track with their life plans and goals. They are welcome to utilize our services at The Club and participate in activities, but there are no requirements.

Supportive House

  • 10 rooms with bathrooms

  • Full communal kitchen

  • Common space

  • Outdoor Patio

  • Large yard

  • Case Manager on-site during the day Monday - Friday

  • Residential Counselor in the evenings and weekends


Community House

  • 12 apartments: 8 studios; 4 one-bedrooms

  • All apartment kitchens have a stove, sink, refrigerator/freezer, countertops and cabinets.

  • Laundry Room

  • Common space

  • Outdoor porch

  • Case Manager on-site during the day Monday - Friday

  • Light Supervision

Availability

Pathways Supportive and Community Housing are currently at capacity. Applications will be accepted when space becomes available.

Pathways Housing Needs Assessment

If you or someone you know is in need of housing, please let us know by filling out our “I Need Housing Form.” This form is an ongoing needs assessment survey that will help provide us with a greater understanding of the housing needs for those with mental illness in our community and help us plan for the future.
